Call any office or send the form. If you need to change a visit, tell us as early as you can, so another patient can use the time.
Plan on about an hour for a full exam. If you are running late, call the office, and we find the best way to see you.

Your prescriptionWe keep your records private and share them only as the law allows. Your records are the same at every office.
