What you send us

When you fill in a form, we receive the details you type: your name, phone number, email, the office you prefer and your message. We use them to call or write back to you about a visit.

Please don't put medical details in a form. For anything urgent, call us.

Your records

We keep your records private and share them only as the law allows, for example with your plan to bill a visit or with an ophthalmologist when we refer you. Your records are the same at all four offices.
